Two thirds of school age children in State care have no records of health or medical assessments while three quarters did not have a documented education plan, a damning report into children in care has found.

A report by the Ombudsman Western Australia investigating planning for children in care of the Department for Child Protection’s chief executive, tabled in State Parliament yesterday, found many of the children included in its investigation had not receive appropriate health care and education planning.
